I have not had any Covid-19 Symptoms in the last 14 days and have not been told to self- isolate.

I understand Clay Pigeon Shooting is a sport that has inherent risks such as falling clays, firearm recoil, projectile ricochet, uneven farm land, hearing damage and accidental discharge. I understand that there may be a risk of injury or death by participating in this activity. I have read and considered the risk assessment at www.coombsclayshooting.com/riskassessment/

I shall wear Personal protective equipment at all times such as ear plugs or ear defenders and safety glasses in and outside of the stand.

I am not under the influence of drugs, alcohol or any other substances controlled or uncontrolled including medication that may affect my mental and physical ability. I am of sound mind and currently feel OK in terms of my mental heath.

I am generally in good health and have no conditions, illnesses or ailments that may be worsened by the activity or may even compromise my ability to operate a firearm safely.

I am not a prohibited person as defined below: It is an offence for a person who is prohibited by Section 21 of the Firearms Act 1968 to have a firearm or ammunition in his or her possession at any time. This section applies to any person who has been sentenced to imprisonment or to youth custody or detention in any young offenders institution for three months or more. A person sentenced to a period between 3 months and 3 years is prohibited for 5 years from the date of release. A person sentenced to 3 years or more is prohibited for life. This includes suspended sentences.
